My Health Planner is a user-friendly app that facilitates communication between patients and their care teams. With just a few taps on your iPhone or iPad, you can conveniently message your care team whenever you need. This feature ensures that you stay in control of your health and have a clear understanding of your personalized care plan.

To further enhance your experience, My Health Planner allows you to stay connected with our care management team between appointments. You can set medication reminders, monitor your progress, and track your daily steps. By prioritizing your privacy and security, we ensure that our app is fully compliant with HIPAA regulations.

Best of all, My Health Planner is available free of charge for eligible members as part of their benefits package. Simply sign up using the access code provided by your care team, along with your insurance card.

 <li>Who can use this app:</li>
 <li>If you are a member of BlueCross BlueShield of South Carolina, BlueChoice Health Plan, or the Blue Cross and Blue Shield Service Benefit Plan (FEP ®), you can take advantage of this app.</li>
 <li>If you have a different BlueCross plan, please check if "My Health Toolkit" is included on the back of your insurance card's website.</li>
 <li>This app supports all medical and dental benefit plans administered by BlueCross BlueShield of South Carolina and BlueChoice Health Plan. It also works with select large employer plans administered by affiliated Blue Plans such as CareFirst BlueCross BlueShield and Excellus BlueCross BlueShield.</li>
</ul>

Please note that My Health Planner is not compatible with Medicaid members. It does, however, integrate seamlessly with Apple Health for certain care programs that involve step count monitoring.

It's important to consult with a healthcare professional before making any medical decisions. My Health Planner is designed to provide educational resources and support between provider visits. While enrolled in an eligible care program, you may connect a glucometer to share readings with your care management team. However, it's crucial that you consult your doctor before making any changes to your diet, exercise routine, or treatment plan.
